Care Coordination Program

The care coordination program provides services for members with short-term and/or emerging needs.

The program helps you with:

  • Access to pharmacy support, durable medical equipment (DME), discharge and transitional planning needs, dental access issues, referrals and access to medical and behavioral health care providers or specialists, and community resources
  • Getting answers to questions about how to obtain medications, supplies, or medical equipment
  • Finding a health care provider or specialist and making your health appointments

You have the right to choose not to participate and opt out of any program at any time. To opt out, please call 1-866-577-0833 (TTY 711). You do not have to participate in the program to continue to receive services from the plan.
